Job Summary and Qualifications The Transplant Clinical Nurse Coordinator (CNC) is a hybrid Charge RN/Leadership role that functions to deliver high quality, patient-centered care while overseeing the coordination of department processes. In collaboration with other members of the team, the CNC directs, monitors, and evaluates nursing care in accordance with established policies/procedures, serves as a clinical resource person for patients and bedside staff, and models a commitment to the organizations vision/mission/values to support and maintain consistently excellent patient experience and quality clinical outcomes for a specialized patient population.What you will do in this role:This is a permanent Charge RN position with additional office hours for leadership responsibilities.
  • Assists with admission and discharge processes to ensure efficient throughput and high quality, patient-centered care.
  • Participates in the ongoing assessment of the quality of patient care services provided in the unit, in collaboration with other members of the management team.
  • Collaborates with staff daily to support a positive environment of teamwork that results in quality patient outcomes, service excellence, and staff satisfaction.
  • Supports a patient-first philosophy and engages in routine Nurse Leader Rounding, providing service recovery when necessary.
  • Supports the efforts of the facility to improve engagement by operationalizing current nursing strategies, including employee rounding, employee recognition, and evidence based department process improvements.
  • Provides recommendations related to interviewing, onboarding, and training new staff. Recommends and implements courses of action, including training and development, conflict resolution, personnel policy compliance, completion of performance evaluations, and/or disciplinary actions to ensure clinical excellence.
  • May assist with staff scheduling and day-to-day staffing; Manages all practical aspects of staff labor in accordance with patient care needs and established productivity guidelines.
  • Supports proper inventory control and assists with managing supplies and equipment in a fiscally responsiblemannerClinical Nurse Coordinator Qualifications:
    • Two years or moreclinical/in-patient experience with intermediate level of care patients required
    • Charge Nurse experience in an inpatient setting required
    • Preferred: Previous experience with solid organ transplant patients in an inpatient setting.
    • Graduate of an accredited school of nursing. BSN strongly preferred.
    • Current licensure in the State of Colorado as a Registered Nurse, or current active multistate Registered Nurse license.
    • Basic Cardiac Life Support (BCLS) and Advanced Cardiac Life Support (ACLS) must be obtained within 30 days of employment start date
